Please provide the following documents upon submission:

FINANCIAL STATEMENTS

  • CURRENT STATEMENT fiscal year end, six month interim, or quarterly report, whichever is most current.
  • FISCAL YEAR END REPORT for the last three years.
  • LAST FISCAL YEAR END on any other business in which any owner has 20% or more ownership.
  • CURRENT PERSONAL STATEMENT for each owner.
  • RESUME OF EXPERIENCE for each owner, partner, or stockholder; and key employees, if applicable.

    In addition, you must complete the following CBIC forms included in our Standard Market Program Kit:

  • Application/Questionnaire for Contract Bonding**
  • Schedule of Uncompleted Work**
  • Bond Request (If final bond, please provide a copy of the contract.) (See Documents in Download Center)
  • Business Summary
  • Bank Authorization Letter

    ** Substitute forms from other recognized surety companies will be accepted. After reviewing the underwriting documents listed, a CBIC Contract bond underwriter will contact you personally to discuss suretyship for you and/or your client. After that discussion, we may request additional items to complete the underwriter's file. If everything is acceptable, a specific CBIC commitment to provide surety bonds may then be made.
